Re: Question about pain and gastritis

Hi after a normal work up the main strategy for this kind of symptom is to exclude anything serious or easily treatable and then expect symptoms to gradually improve, especially in a young person. I don’t encourage the diagnosis of stress as a cause of organic disease because it is pretty much a “de...

Re: Low libido

Both drugs but more likely the second could temporarily affect erectile function and/or libido. It’s in the fine print. I suggest you cease acotiamide to see. You could also switch ranitidine to omeprazole 20mg once or twice daily. This is really a discussion for your family doctor. It sounds a bit ...

Re: Nexium Hp7 triple therapy

Its confusing and impossible to work out exactly, but I kind of agree with your doctor. Just take all the antibiotics you have, don't forget the Nexium (more is better so I often give 40 mg 3x daily). If you fail treatment then nothing is lost because it means the bacterium was resistant to clarithr...

Re: Lyme like illness

Regards Lyme: The ability to pull out PCR DNA fragments from a biological sample is not necessarily proof that the germ exists. Bacteria swap bits of DNA around so a piece could resemble a Lyme DNA yet Lyme might not be present. Similarly, antibodies which show up against Lyme could actually be just...

Re: Just started H. Pylori Treatment... and POLYPS

Gastric polyps of about 5 mm size are actually quite common (5-10% of people having endoscopy) and are almost always benign. So my strategy is to biopsy one or two so that I can tell the patient that the polyp IS benign. Some say that's a waste of money because it is so rare for a benign appearing "...

Re: How should I do to avoid transmission?

These days, in a modern country with clean chlorinated water supply, Hp can only be acquired from a close family member. That means a spouse or sometimes mother to child.
So I recommend have your partner tested and then both take the treatment at the same time. Follow up with breath tests. 8-)

Re: PBRC treatment Dosage Intervals

Spreading out the doses does make sense. However, it is not proven to make any difference. In fact, in one study, two doses per day given at mealtimes was just as effective. Also, separating one type of tablet from the other, is unnecessary. If anyone knows of true evidence on these points let me kn...

Re: 2nd line of treatment? Diagnosis in people needing PPI

An area of management worthy of discussion. Firstly, every (DIRECT) test becomes less accurate as we interfere with the stomach environment. In order these effects are: antibiotics > bismuth > PPI > H2-Blockers > antacid. So tests which measure actual bacteria in the stomach (biopsy, culture, breath...

Re: 4th Line Treatment - Rifabutin and Neutropenia

Final Reply Neutropenia is somewhat idiosyncratic which means people who have a reaction to the rifabutin would likely develop it. As I review your past treatments my comments are: 1. Repeated treatments with clarithromycin or metronidazole are not very useful. 2. Since the acid blocker is inexpensi...
